We made hourglasses this way:

Two plywood rounds with four flanges spaced evenly around the edges.  Placed 
1/2" emt (conduit) into the bottom round.  Inflated  2  - 3 foot clear 
balloons.  Once we placed grains of sand (5" ivory). We tied the two balloons 
together.  Added the top round.  We painted the rounds and the conduit.  But, 
you could add 5" balloon garland on the EMT.  You could make a miniature 
version using 5" balloons and foamboard the centerpieces.
