Subject: Need a donation form

  It must be that time of the year again, but it seems that it's always that 
time of the year when the phone or mail bring a request that "We would really 
love a donation of some balloons from you."
  I have several charities to whom I do "complete donations" yet it is 
impossible to do so for everyone.  
  I did check the Balloon HQ Archives and know that I am already following 
the same policies as Lindy and Wynn Bell, Rocky Toomey and Mark Balzer (not 
bad company), but I feel the need for a new Gift Certificate form and was 
unable to locate one in the Archives.
  This would be for a donation of "X amount" of balloons to be picked up by 
the person (or organization) with an expiration date for the time of the 
certificate being valid. 
   If you have one that you would be willing to share, I would be most 
appreciative.
